Documents needed for FSSAI State Licence
Keep these ready. You upload them in step 2; items marked "if applicable" are needed only in some cases.
Identity proof
- Passport-size photograph
- Photo ID of the proprietor, partners or directors
Address and premises proof
- Business address proofElectricity bill, property tax receipt or municipal khata, not older than 2 months.
- Rent agreement or owner's NOC (if applicable)Needed if the premises are rented or owned by someone else.
- Layout plan of the premises (if applicable)Needed for manufacturers.
Business documents
- List of equipment and machinery (if applicable)
- Water test report (if applicable)Needed for manufacturers and restaurants.
